Aaron Gustafson, LMT, CAMT II

In-network provider for Aetna. Not currently offering on-campus sessions, please come see me @ my SE Clinton office.

*Aetna benefits apply for your treatment when you have a Rx for massage. (Intake form/chart note required for each session.), co-payment or co-insurance %, and deductibles apply. Relaxation massage is not billable to insurance; you must have a prescription for massage from your physician.

Getting a prescription can be easy: A call to your doc’s office may be all it takes to get your prescription; or they may want to see you for an office visit first. If you are managing musculoskeletal pain of any kind, even a sore neck or shoulders that come from the work week, most physiscians are happy to write you a Rx for massage therapy. Your Rx must include a diagnosis code (such as neck pain, back pain, etc.), total # of sessions, and the period of time the sessions should be administered over, e.g.: 1 to 2 massages per month for 12 months. Prescriptions may be faxed to 503-300-5466, emailed, or brought in person.

If you have questions about your massage benefits, please contact Aetna at 1-800-826-2386. Be sure to ask how many sessions per year you have, how many remain, and what your co-pay or coinsurance amount is

I must verify benefits prior to your appointment. Please send a picture of both front and back of your ID card and send your birthdate. You may email or text them to 503-407-6046, verification takes 1-3 business days. Verification is only for those who have booked an upcoming appoinment.
